I hereby voluntarily give my consent to Edward Belacse Career Consultancy Services A.K.A Olympuz ("the Agency") to collect, record, organize, store, update, retrieve, consult, use, consolidate, process, share, block, erase, or dispose of my personal information and other personal data that I have provided or may provide in connection with my employment or engagement with the Agency.
I declare that the personal information I have disclosed is true, complete, and accurate to the best of my knowledge. I understand that such information is collected and processed by the Agency for legitimate business purposes, including but not limited to recruitment, employment administration, payroll processing, benefits administration, statutory reporting and compliance, employee welfare, training, performance management, and other purposes necessary to fulfill the Agency's legal and contractual obligations.
I understand that my personal information shall be retained by the Agency for the duration of my employment or engagement and thereafter for such period as may be required or permitted under applicable laws, regulations, and the Agency's records retention policies.
I acknowledge that the Agency shall implement appropriate organizational, physical, and technical security measures to protect my personal information and shall process such information in accordance with the Data Privacy Act of 2012, its Implementing Rules and Regulations, and other applicable data privacy laws and regulations.
I further authorize the Agency, and where necessary, its authorized personnel, service providers, affiliates, government agencies, clients, or other third parties, to process or disclose my personal information only for lawful purposes, including compliance with legal, regulatory, contractual, or governmental requirements, or when required by a lawful order of a court or competent authority.
I understand that providing my personal information is generally necessary for the processing of my employment or engagement with the Agency. I also understand that, subject to applicable laws and contractual obligations, I may exercise my rights as a data subject, including the right to access, correct, object to, or withdraw my consent to the processing of my personal information by submitting a written request to the Agency's designated Data Protection Officer or authorized representative. I acknowledge that the withdrawal of consent shall not affect the lawfulness of any processing conducted prior to such withdrawal and may affect the Agency's ability to continue my employment or provide certain services where the processing is required by law or contract.
I hereby declare that I have read and understood the contents of this Declaration and Consent, and that I am voluntarily giving my consent without force, intimidation, coercion, or undue influence.
